Website Form Integration

Hi,

Suppose I have a " contact us " button on every page of my website ( in the static header section ) which is a pop up, how do I integrate this form to Eloqua ?   Also if a download / white paper has a separate form to be filled up for the download on a few of the pages how do I track this?(basically how do I track downloads)
